• Client Consent Form - this form can be filled out by opening the file, fill in the fields, print, allow Client to read and sign.  Once signed scan or take a picture of the signed document and attaching to the Client Entry page.

• ​Agency Agreement Form - can also be filled out by opening the document, print, read and sign the document.  Once signed by both parties, scan and upload the document by attaching the appropriate Agency in CCBC
